Plan Options

The plan elected will determine the benefit amount applied. Plan options include:

Plan 1 – Up to $2,500 per Covered Accident; $100 deductible applies per covered accident Includes $2,500 Accidental Death & Dismemberment Principal Amount

Plan 2 – Up to $5,000 per Covered Accident; $100 deductible applies per covered accident Includes $5,000 Accidental Death & Dismemberment Principal Amount

Plan 3 – Up to $10,000 per Covered Accident; $250 deductible applies per covered accident Includes $10,000 Accidental Death & Dismemberment Principal Amount

The AD&D and Accident Medical Expense benefit insurance is subject to the terms, definitions, conditions, exclusions and limitations of the group policy. These insurance benefits are underwritten by Guarantee Trust Life Insurance Company. These benefits are under policy Form MP-1300 issued to the American Advantage Association. Coverage becomes effective on the date provided in your membership materials. Guarantee Trust Life Insurance Company (GTL) provides the Accident medical coverage and Accidental Death and Dismemberment coverage. GTL does not provide nor is affiliated with the discount programs provided as a part of the membership in American Advantage Association.
